Contract Completion Instructions

To complete the Account Opening Contract at Pinetree, please follow these instructions:

Way 1: Visit Pinetree in person to complete the contract.

Way 2: Send the documents via express mail.

The documentation set includes:

• Securities account opening contract: 2 copies
• Valid ID Card/Citizen ID: 1 photocopy
• Sample form for reference: Account Opening Contract (Foreinger Individual)

Important Notes on Account Opening Contract Information:

1. Personal Information:
   • Full name, date of birth, gender, nationality, ID number, date of issue, place of issue, permanent address, contact address, phone number, email.
2. Bank Account Information:
   • Account number, bank name, branch.
3. Customer Signature:
   • A total of 3 signatures for opening a regular securities trading account and 4 signatures for opening a margin trading account.

Important Notes for Registration at Pinetree:

1. Account Registration:
   • On page 2 of the contract, in the account registration section, the customer should tick “Yes” and sign once for a regular securities trading account, or twice when registering for a margin trading account.
2. Bank Account Registration:
   • Customers can pre-register up to 2 bank accounts to transfer money from their securities account to their personal account.
   • (Customers can add up to 3 more bank accounts online.)
3. Service Registration:
   • Please tick “Yes” for the services you wish to register for.
4. Authentication Method Registration:
   • Besides SmartOTP and receiving OTP via phone messages, customers can also register for Matrix authentication.

Additional Notes for Account Opening Contract:

• The contract must not be torn, dirty, or erased.
• Information on the contract must be accurate and consistent with the information on the ID/Citizen ID.
• Use a consistent signature, sign, and clearly print your name in the designated customer signature boxes.
• Please fill out the contract using blue ink.
